What to bring to your first appointment

Completed Health History Form
Bring a printed copy of your completed health history form, or submit it as directed when you speak with our office.

HIPAA Form
Bring your signed HIPAA-related paperwork as provided by our team.

Release of Records
If you are transferring from another dental office, bring a completed release of records form so we can request your prior records.

Insurance information, if applicable
Bring your insurance card and any subscriber or group information so we can verify your benefits.
